This 40 credit degree-level module will provide contemporary knowledge, skills and competencies for practitioners who wish to advance their practice to effectively manage a range of long-term conditions.

The module will highlight the significance and application of links between research/audit and evidence-based care in the management of long-term conditions and will equip you to lead and implement informed change in the practice of long-term conditions management.

At the end of the module you will be able to:

• demonstrate a critical understanding of case management and its application to patients with multiple, complex needs.
  
• systematically apply the principles of clinical assessment of patients including the ability to take a thorough history and carry out a physical examination of the cardiovascular, respiratory, gastrointestinal, and neurological systems, identifying abnormalities and offering a rationale for your actions.
  

Delivery consists of a variety of approaches including: lectures, group work, student-led activities, expert patient, simulation, equipment management, practical scenarios, digital media and directed study.
